Please indicate if you want us to refer this accident to our claims attorney. He will file your claim for you against the other party and try to have a check for you within the month!

 

Note: Download the MV-104 form and fill out as much information. Submit the completed MV-104  form together with your police report and photos of your vehicle accident on the form below. Your Lone Star Brokerage Representative will begin initiating this claim.
